For a number of years, hair transplant surgeons have used either metal staples or, more commonly, a running suture of Surgilene (or Nylon) to close the donor wound. However, neither type of closure was completely satisfactory.
Metal staples were uncomfortable and disliked by many patients and they often left a fine, but very distinct line in the donor area. On the other hand, Surgilene sutures (made of a non-absorbable, synthetic, mono-filament) were more difficult to remove. More importantly, when there was any wound tension, or if there was significant post-op edema, the running suture could strangulate follicles and result is localized hair loss around the suture line.
To minimize any potential loss of the hair that was incorporated within the running suture, hair restoration surgeons began to place the sutures closer to the wound edge. Although this minimized the amount of trapped hair, it also made the sutures even more difficult to remove, as they became buried within days after the procedure.
In an effort to produce the best and most comfortable results for the patients, hair transplant surgeons have tried using various absorbable sutures, such as Chromic and Vicryl. Unfortunately, these were also not perfect solutions, producing too much tissue inflammation. The fact that they did not need to be removed did not offset the increased risk of follicular damage from the inflammation.
A New Suture
At the American Academy of Dermatology Annual Meeting in 1997, Johnson and Johnson introduced a new absorbable suture called Monocryl. The suture, made of Poliglecaprone 25, a synthetic, monofilament suture was touted as being easy to tie, very strong and preserving most of its tensile strength for up to three weeks post-op. Most importantly, it was broken down by hydrolysis rather than needing an active inflammatory response of the body to degrade the suture. This seemed to be the answer to the problem.
Hair restoration centers started using 3-0 Monocryl in a running stitch, placing it relatively close to the wound edge. Doctors quickly learned that 4-0 and even 5-0 was sufficiently strong to hold the entire wound together. With the finer sutures, hair transplant surgeons could place the stitches as close to the wound edge as 1.5 mm and still obtain a secure closure.
One of the tricks doctors learned was that advancing the suture on the surface rather than under skin (as surgeons traditionally did) had two advantages. It minimized the amount of suture that crisscrossed the follicles under the skin and allowed the clipped hair at the edge of the wound (that the suture did cross over on the surface) to keep the sutures from becoming buried too quickly -- if at all. This new suture and suturing technique soon became the closure method of choice.
The Study
To test the usefulness of this new technique, hair transplant surgeons conducted a bilateral controlled study comparing staples to the new Monocryl sutures. Although 4-0 sutures were used in the study, the research also used the finer 5-0 as the work-horse diameter, as this allowed the most precise control of the wound edges and the least tissue reactivity.
In the study, hair transplant doctors made objective measurements on the dimensions of the resulting donor scars and took subjective responses from the patients in the study. The two groups were evaluated with regard to healing, post-operative discomfort, resultant surgical scar, and closure material preference.
The average scar width on the staples side measured 1.78mm compared to a 1.42 mm on the sutures side. Fourteen of the 22 patients in the study preferred Monocryl for future procedures; one preferred staples and 7 had no preference. Of those that preferred sutures, post-operative discomfort from the staples and the inconvenience and occasional pain associated with their removal was responsible for their decision.
Based on this study, many hair transplant surgeons now use Monocryl sutures for the majority of donor incision closures. However, some cases occasionally require the use of staples in select patients, particularly in those with very high hair density and loose scalps.

Hair Transplants Woman hair loss has been the subject of many studies, treatment options for treatment of woman hair loss continued to be unclear. Follicular Unite Transplant (FUT) set a new standard in hair restoration surgery. FUT has made the dream of natural looking hair transplant, a reality. Women with hair loss could be candidate for follicular unit transplant as it is currently the standard of care in hair transplant surgery.
Other advances in techniques and instruments for hair transplant made larger surgeries possible for more sever hair loss for men and women and facilitated the transplant of a maximum number of grafts in only one session. The emphasis on natural looking surgery and elimination of noticeable scars advanced the techniques further.
Medications such as minoxidil could be used along with hair transplant for women. Minoxidil can help thicken miniaturized hair in many patients, prevent or slowed hair loss in women in early stages of hair loss. Minoxidil may be synergistic with hair transplant in preserving and/or reversing hair loss.
The future is bright with the promise for hair multiplication over the coming years. Hair Transplants In follicular unit hair transplantation, donor hair is harvested by excising a narrow strip of scalp from the donor area—an area at the back of the head that is resistant to hair loss. Each follicular unit in the donor strip contains one to four hairs. Follicular unit transplantation is extremely efficient because several thousand hair grafts can be obtained from one donor strip. While in principle follicular unit hair transplantation is very simple, its implementation requires meticulous micro-surgical skills, attention to detail, and artistry.
Hair in the scalp grows in small family groupings know as follicular units, which contain between one and five hairs each. It is important that the integrity of these groupings remain undisturbed during the dissection process.

When hair transplantation first came onto the scene, men and women with thinning hair or bald scalps (who had been hiding under their hats and scarves for years) finally had some hope. Those who could afford this “new” technology ran to the first doctor who claimed they could cure their baldness.
Unfortunately, the technology, which seemed advanced for the time, could often result in pluggy, unnatural looking hairlines. For many who had these “baby doll” transplants, the results were both cosmetically and psychologically disastrous.
Older methods of hair transplantation included punching out circles of hair-bearing scalp that were then moved to the balding areas. Practices such as scalp reductions (literally cutting out the balding areas of scalp and raising the two sides of hair-bearing scalp toward the center) often left long visible scars. For a time, there was also the method of planting synthetic hair directly into the scalp and then surgically sewing it beneath the skin, a procedure that is now illegal in the United States. This latter was a short lived experiment which caused the hair to be rejected and the scalp to be severely scarred.
Problems with the Surgical Procedure
The older surgical procedures presented numerous cosmetic problems, the most common of which were the large, unnatural look of plug-grafts. These punch graft techniques of hair transplantation also tended to leave circular scars throughout the donor area (the donor area is the area of scalp where the hair is harvested from for the transplant). Other problems included poor hair direction, inconsistent hair growth, and a generally unnatural look to the hair restoration procedure. In many cases the results were so bad that patients were left wishing they were bald again.
From Old to New
With the introduction of today’s state-of-the-art techniques there are now ways of not only restoring hair in a natural way, but also of repairing and redesigning the results of the older techniques. Procedures such as Follicular Unit Hair Transplantation (FUT) are able to address the issue of transplanting hair without the consequence of unsightly scarring.
Undoing the Damage
Reversing the unnatural appearance of the older plugs and repairing scars is obviously more involved than using the right technique in the first place, but it can be accomplished in most cases with excellent results. Addressing the repair of existing scars requires finesse and careful assessment of the problem area as well as the remaining donor supply. Choosing the correct restorative technique is critical as the wrong strategy can make the situation worse.
Different techniques must be used to resolve different problems. Large plugs can be removed, divided microscopically into individually follicular units and placed back into the scalp in a pattern which is much more natural in appearance. Smaller grafts can be camouflaged. Scars that are raised can often be helped with cortisone injections. These can help to decrease the inflammation which is often present in a raised scar. Sometimes excessive scar tissue can also be excised and removed in itself, particularly when associated with poorly positioned grafts of hair.
The Proper Way to Transplant Hair
A lot of improvements in scientific techniques, medical developments and methods of artistic design have been made since hair transplantation was first introduced. Today’s hair transplant techniques can mimic the way hair grows in nature. The most important advancement is the identification of naturally occurring tiny bundles of hair called “follicular units.” This discovery has been fundamental to the major advances in literally all of the surgical hair restoration procedures used today.
These methods not only enable the physician to produce results that mimic nature but provide the hair restoration surgeon with the tools to actually fix the problems of old unsightly grafts and scars. The number of people who will benefit from hair transplant repair techniques will continue to grow over time as more people become aware that the older procedures can actually be corrected.
